What is menopause and what changes happen?
Menopause is when your periods stop for good. It happens because your ovaries stop releasing eggs and stop making as much estrogen. On average, this happens around age 51.
The estrogen drop is the real driver behind most of the changes women notice. Estrogen does more than regulate periods, it also helps protect bone and supports healthy blood vessels. Once estrogen levels drop, it can affect more than just hot flashes and mood. Low estrogen can cause fall, bone loss speeds up and cardiovascular risk factors like cholesterol and blood pressure tend to shift in a less favorable direction.
Because of this, postmenopausal women face a much higher lifetime risk of bone fracture, roughly a 1 in 2 chance. At the same time, doctors now think of the menopause transition as a "cardiometabolic transition," meaning it's a period where cholesterol, fat distribution (especially around the belly), blood sugar regulation, and blood vessel health all start shifting toward higher risk. Research following large groups of women has confirmed this pattern happens fairly consistently during and after the transition.
This is really the reason nutrition becomes so important at this stage. The hormonal shift is out of your control, but the foods you choose can meaningfully offset some of the added risk to your bones and heart. Below are simple, evidence-based nutrition strategies to support your bones and your heart during and after menopause.
Why Menopause Affects Bone and Heart Health
Estrogen helps protect bone density and keep blood vessels healthy. When estrogen drops during menopause, bone loss speeds up and cholesterol and blood pressure patterns often shift in the wrong direction.

The Best Overall Eating Pattern: Mediterranean Style
Research consistently points to the Mediterranean diet as one of the most helpful eating patterns for women going through menopause.
In a large study following over 90,000 postmenopausal women for almost 16 years, those who closely followed a Mediterranean style diet had a 20 percent lower risk of hip fracture. Other research has linked this eating pattern to major reductions in heart disease events as well.
What a Mediterranean style plate looks like:
Plenty of vegetables and fruit
Whole grains like oats, brown rice, and whole wheat bread
Beans and lentils
Fish a couple times a week
Nuts and olive oil as main fats
Less red meat, less processed meat, less added sugar
The DASH diet, which is very similar and was originally designed for blood pressure. Most studies suggest strong support for bone density and heart health in postmenopausal women.
Key Nutrients to Focus On
Calcium and Vitamin D
These two work together to protect your bones. Combined calcium and vitamin D intake has been shown to lower hip fracture risk significantly.
General targets:
Calcium: about 1,000 to 1,200 mg per day
Vitamin D: about 800 IU per day
Best food sources of calcium:
Dairy products (milk, yogurt, cheese)
Fortified plant milks
Leafy greens like kale and bok choy
Canned salmon or sardines with bones
Food sources are generally preferred over supplements when possible.
Protein
Protein supports bone strength and helps protect muscle mass, which naturally declines with age. Aim for roughly 20 to 25 grams of high quality protein at each main meal.
Good protein sources:
Eggs
Greek yogurt
Chicken, turkey, or lean beef
Fish
Tofu, tempeh, and beans
Fruits and Vegetables
Eating fewer than 5 servings a day is linked to a higher risk of hip fracture. Fruits and vegetables also support heart health by lowering the risk of heart disease and stroke.
Simple goal: Try to fill half your plate with fruits and vegetables at most meals.
Omega-3 Fats
Omega-3 fats, found in fatty fish like salmon, mackerel, and sardines, can help lower triglycerides and support heart health. Walnuts, flaxseed, and chia seeds are good plant based sources too.
Soy and Phytoestrogens
Soy foods like edamame, tofu, and soy milk have been linked to lower cardiovascular disease risk and small improvements in cholesterol. Research on soy and bone density shows a modest benefit as well. Whole soy foods are generally recommended over concentrated isoflavone supplements.

A Note on Supplements
Omega-3 supplements can lower triglycerides, but they may also modestly raise LDL ('bad') cholesterol by about 4 mg/dL in studies of postmenopausal women. This effect seems to come mainly from DHA, one of the two omega-3 fats in fish oil, rather than EPA. The 2026 ACC/AHA guidelines flag this LDL increase as a potential concern with fish oil supplements. This is one more reason to prioritize getting omega-3s from whole foods like fatty fish when possible
Soy supplement concentrated capsules or tablets containing high doses of soy isoflavones, which are plant compounds that weakly mimic estrogen in the body. They may be associated with increased risk of estrogen receptors (ER) - negative breast cancer and increased risk in women with a family history of breast cancer.
Calcium supplements are not automatically the safest choice. Some research has linked high dose calcium supplements to a higher risk of kidney stones and possible cardiovascular concerns. This is another reason food sources are usually the better first choice, with supplements used to fill specific gaps rather than replace a healthy diet.
Frequently Asked Questions
Do I need to cut out dairy during menopause? No. Dairy is actually one of the easiest ways to get both calcium and vitamin D in one food. Unless you have an intolerance or allergy, dairy can be a helpful part of a menopause friendly diet.
Is soy safe during menopause? Yes. Current research supports whole soy foods like tofu, edamame, and soy milk as safe and potentially beneficial for heart and bone health during menopause.
How much calcium do I actually need? Most postmenopausal women need about 1,000 to 1,200 mg of calcium per day, ideally from food sources first.
Can diet alone prevent osteoporosis? Diet is a major piece of the puzzle, but it works best alongside weight bearing exercise, adequate vitamin D, and regular checkups with your doctor. Diet lowers risk, it does not guarantee prevention on its own.
